Experimental Longevity Drug Survey

This survey is run by Vitality Now, a Swiss Verein registered in Zug (c/o MJP Partners AG, Bahnhofstrasse 20, 6300 Zug), the legal entity through which VitaDAO operates, in partnership with Infinita, LARTA, and Wildcreek Life Sciences. Vitality Now is the sole recipient of individual responses. We use your answers, on the basis of your consent, to assess demand for experimental longevity protocols in emerging jurisdictions. Some questions cover medication, supplements and health markers, so please share only what you are comfortable sharing and skip anything you prefer not to answer. We do not ask for your name; the email field at the end is optional, and if you provide it your response is linked to it and is no longer anonymous to Vitality Now. We publish only anonymized, aggregated results and never individual answers or anything that could identify you. Responses are collected and stored via Tally, our form provider. We do not sell your data or share it outside Vitality Now, except with service providers acting on our instructions. We keep responses until December 2027, after which they are deleted or fully anonymized. Participation is voluntary and limited to people aged 18 or over. You can withdraw your response at any time before that date by writing to privacy@vitadao.com. EU, Swiss and UK respondents also have rights of access, rectification, erasure and complaint to their supervisory authority.
